Job Description
Frontline Support Services is seeking a dedicated and experienced Case Manager to join our team in Cairns, QLD. This is a subcontracting role (ABN required), offering an prospect to make a meaningful impact in the lives of children, young people, and families within our community.

As a Case Manager, you will play a key role in coordinating and delivering essential support services to vulnerable individuals and families while working collaboratively within a supportive and professional team environment.
n
Key Responsibilities
n
- Conduct comprehensive assessments to identify client needs and develop tailored support plans
n
- Coordinate a range of support services, including referrals, advocacy, practical assistance, and stakeholder engagement
n
- Advocate on behalf of clients to ensure appropriate supports and resources are accessed
n
- Work collaboratively with internal teams and external stakeholders to deliver holistic, client-focused care
n
- Maintain accurate and detailed case notes and documentation in line with organisational policies and compliance requirements
n
- Participate in regular supervision, team meetings, and ongoing professional development
n
- Participate in an afterhours on-call roster shared within the case management team
n
About You
n
- A relevant tertiary qualification in Social Work, Psychology, Human Services, or a related field (or currently enrolled)
n
- Minimum 2 years' experience in a Case Management or similar role within the community services sector
n
- Strong interpersonal and communication skills with the ability to build rapport and professional relationships
n
- Demonstrated ability to manage complex situations and apply critical thinking and problem-solving skills
n
- Knowledge of child protection legislation, reporting obligations, and trauma-informed practice
n
- Ability to work collaboratively within a multidisciplinary environment
n
- Excellent organisational and time management skills with the ability to manage a varied caseload
n
- Current driver's licence, valid Blue Card, Yellow Card/NDIS Worker Screening, and ABN
n
