Tradewind Australia is partnering with a community-based Domestic & Family Violence support service to recruit for a compassionate & resilient locum practioner within the Mackay region.

Position Overview

n

This position will provide place based and outreach case management support to women and women with children who have experienced and/ or are currently experiencing family violence. The role also encompasses the delivery of assessment and crisis response.

n

The role will be in instrumental in delivering high quality, secure and effective family violence and child well-being responses to those seeking support, safety, and recovery.

n

ROLE RESPONSIBILITIES

n

n
- Effective relationships are established with clients that encourage self-determination and participation and uphold the principles of the National Family Violence Model of Care.
n
- Case management support is provided to clients in accordance with TSA case management models and standards of practice
n
- Ongoing assessment is made of the client's issues, needs and circumstances from initial intake to exit
n
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
n
- Case plans are implemented and actively led by the participant
n
- Clients are assisted to access holistic support services and establish community connections, and where needed are assisted through advocacy with service providers
n
- Progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals
n
- Accurate, up to date client records and data is maintained for all clients in accordance with TSA standards and procedures using TSA information systems
n
- Risks to clients, staff and TSA are identified and bought to the attention of the Team Leader as soon as possible
n




- Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader
n
- Key performance indicators are met in accordance with outcome measurement timeframes
n
- Participation in an on-call roster
n

QUALIFICATIONS AND EXPERIENCE

n

n
- A tertiary qualification in Social Work or equivalent (Human Services, Psychology, Graduate certificate in Family Violence) is essential.
n
- Experience working in the family violence sector is desirable.
n
- Experience in specialist family violence and/ or child wellbeing work is highly desirable
n
- Demonstrated case management experience and skills
n
- Highly developed time management and work prioritisation skills with an ability to work autonomously with minimal supervision required
n
- Advance computer skills including experience in data and record management
n

REQUIREMENTS OF THE ROLE

- Relevant child protection requirements are obtained - Blue Card QLD
n
- A national police record check is required
n
- Evidence of qualifications
n
- Current Queensland Drivers Licence
n
- Compulsory Code of Conduct, WHS and Bullying and Harassment training modules are completed and up to date.
