About the Role
n
We are urgently seeking experienced and compassionate Disability Support Workers to join a Supported Independent Living household in Zillmere.
n
You will support two participants with different health and daily living needs, including muscular dystrophy, sleep apnoea, heart-related conditions, diabetes requiring insulin management, pancreatitis and communication difficulties.
n
This role requires workers who are confident providing personal care, medication assistance, diabetes support and safe manual handling. Morning, afternoon and sleepover shifts are available.
n
Key Responsibilities
n
n
- Provide respectful assistance with personal care and daily living activities.
n
- Support safe transfers and mobility using a Sara Stedy transfer aid.
n
- Assist with diabetes management and insulin administration in accordance with relevant training and care plans.
n
- Provide medication assistance safely and accurately.
n
- Monitor participants' health and wellbeing and promptly escalate concerns.
n
- Follow individual care plans, clinical guidance and risk-management strategies.
n
- Complete accurate progress notes, medication records and incident reports.
n
- Support participants to maintain their dignity, independence and personal choices.
n
- Maintain a safe, clean and comfortable home environment.
n
- Communicate effectively with participants, colleagues and the wider support team.
n

Essential Requirements
n
n
- Certificate III or IV in Individual Support, Disability, Ageing or a related qualification.
n
- Previous experience supporting people with complex health or physical support needs.
n
- Current diabetes and insulin-management training, or willingness to complete it before undertaking relevant duties.
n
- Current medication-assistance training.
n
- Current manual-handling training and confidence using transfer equipment such as a Sara Stedy.
n
- Experience providing personal care.
n
- Current NDIS Worker Screening Clearance.
n
- Current First Aid and CPR certificates.
n
- Current Australian working rights.
n
- Availability for a combination of morning, afternoon and sleepover shifts.
n
- Strong communication, documentation and teamwork skills.
n
- A reliable, calm and person-centred approach.
n

A current driver's licence and access to reliable transport will be highly regarded.
n
