Job Description
Case Worker - Immediate Housing Services
n
This 12-month contract opportunity (date ending 30/07/2027) is part of the Immediate Housing Response (IHR) team (within the Housing and Homelessness Stream) this position will support singles, couples and families who are experiencing homelessness into short-term and temporary accommodation. The IHR team supports individuals, couple and families via a case management framework to identify and work towards long term housing solutions. As a Case Worker you will support clients within their hotels / motels for the duration of their stay whilst ensuring they meet the Immediate Housing Response criteria and guidelines.

Duties & Responsibilities
n
n
- Provide outreach support whilst working within case management framework to provide culturally safe support to clients who are experiencing homelessness
n
- Conduct intake and comprehensive assessments to identify risks, strengths, and barriers to long-term housing. Work to understand client needs and work with them to create individualised case plans and risk assessments
n
- Develop and implement individualised support plans that address housing stability, financial management, and wellbeing goals.
n
- Provide assistance with household budgeting, referrals, information on housing pathways, advocacy and any other supports identified during engagement.
n
- Support clients to build the necessary life skills and knowledge for a successful tenancy.
n
- Liaise and advocate with landlords, property managers, government departments, and community service providers to support positive outcomes.
n
- Complete referrals to other programs and services and collaborate with a range of agencies and stakeholders to achieve best outcomes for clients.
n
- Support clients to access relevant services such as mental health, AOD, financial counselling, and family support programs.
n
- Maintain accurate case notes, documentation,
and data reporting in line with organisational and funding requirements.
n
- Include clients in decision making, encourage feedback and provide opportunities for input.
n
- Contribute to a positive, cooperative team culture that promotes dignity, empowerment, and client-centred practice.

To be successful you will need
n
n
- Tertiary qualifications in Community / Human Services
n
- Prior experience working with clients from diverse backgrounds and in providing support, case work and or practical assistance to people experiencing homelessness and or a crisis.
n
- Demonstrated ability to establish rapport with people experiencing homelessness, distress, disconnection from family, domestic/family violence, alcohol or other drug challenges and or mental health issues.
n
- Experience in risk assessment, crisis management, incident reporting and skills in navigating solutions to challenges.
n
- Ability to coordinate / deliver activities that support development of practical, relational and or tenancy related living skills.
n
- Ability to collaborate with homelessness, health and medical services, First Responders and other service providers.
n
- Good communication, numeracy, administration, data entry and computer skills and the ability to learn IT systems.
n
- Ability (and willingness) to comply with policies, procedures, established practices and contract deliverables.
n
- Commitment to diversity, inclusion, reconciliation and closing the gap objectives and experience working with First Nations people, organisations and communities.

- Evidence of relevant Tertiary Qualifications Community / Human Services
n
- Satisfactory outcome of a current (less than 3 months old) National Criminal History Check
n
- Blue Card (QLD working with children card)
n
- Current QLD drivers' licence 'Class C' or interstate equivalent
n
- Out of hours work may be required from time to time
n
- Required to transport clients and or to visit clients in their place of residence or within another setting.
n
- Ability to undertake local area / regional travel is required.
n
- First Aid certificate and CPR certificate.
n
