ABOUT THE ROLE
n
Due to the continued growth of our business, we are seeking to hire a full-time Community Support Worker to join our team on an ongoing basis. Reporting to the Community Support Coordinator and Director, the successful candidate will play a key role in delivering direct, person-centred support to NDIS participants in their homes and communities.
n
KEY RESPONSIBILITIES
n
n
- Assessing clients' needs to plan, develop and implement educational, training and support programs, with the guidance and approval of the NDIS Coordinator and Director.
n
- Interviewing clients to assess the nature and extent of services they need, and reporting to the Director and Community Support Coordinator.
n
- Providing direct in-home and community-based support to NDIS participants, including assistance with daily living, personal care, and social engagement.
n
- Supporting clients in attending medical appointments, community activities, and recreational programs to enhance their independence and wellbeing.
n
- Assisting participants to develop skills for daily living, such as cooking, budgeting, and personal organisation.
n
- Monitoring participants' progress towards their NDIS goals and reporting observations to the Coordinator and Director for care planning purposes.
n
- Implementing person-centred support plans that respect the dignity, cultural background, and preferences of each participant.
n
- Liaising with families, health professionals, and other service providers to ensure a holistic approach to client care.
n
- Encouraging social inclusion and community participation to improve clients' quality of life and mental health.
n

Skill & Experience
n
ESSENTIAL CRITERIA
n
n
- A minimum of 2 years' work experience as a Community Support Worker within an NDIS-approved organisation.
n
- A qualification in Certificate III in Ageing and Disability Support, Certificate IV in Ageing/Disability Support, and a Diploma in Community Services.
n
- Exceptional organisational skills and the ability to empathise and connect with people.
n
- Current Working with Children Check and National Police Check (or willingness to obtain).
n
n

- An energetic and motivated individual with a natural flair for working with people with disabilities.
n
- Willingness and availability to work a rotating roster.
n
- Solid multitasking and problem-solving skills, with the ability to quickly learn new concepts and work well under pressure.
n
- A focus on customer satisfaction, with creative thinking, leadership skills, and sound decision-making ability.
n
