Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

About the Role
n We are looking for a motivated Health and Wellbeing Worker who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander people. The successful candidate will provide a high-quality service along with contributing to the development of models of holistic health incorporating continuity of care. As part of Kalwun's ongoing growth and development, you will be joining a community-focused team at the Coomera clinic.

Position Duties
n
n
- Connecting and yarning with clients and families to the service to collect, collate and update their stories
n
- Provide health information with education to clients and families which promotes autonomy and choice
n
- Work alongside clients and families in their goal setting and prioritisation of health and social needs ultimately implementing deadly plans
n
- Responding to clients and families at times of heightened need to prevent and/or avert episodes of crisis
n
- Make a positive contribution to illness prevention and promotion of health through chronic disease management, health screening, health assessments, recalls and patient education.
n
- Relevant Certificate III Qualification (Aboriginal and Torres Strait Islander primary health care, Community Services, Allied Health Assistance or equivalent)
n
- Demonstrate an understanding of issues affecting Aboriginal and Torres Strait Islander people and their health
n
- Ability to communicate sensitively and to effectively engage with community built on mutual respect and regard
n
- Demonstrated ability to meet targets and performance outcomes
n
- Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people
n
- Demonstrated computer literacy including proficient keyboard and data entry skills
n
- Ability to work effectively as part of a multi-disciplinary team
n
- Current Qld C class drivers licence.

- This position requires the successful applicant to satisfy a national criminal history check and hold a current drivers' license.

- A Blue Card for working with children is a mandatory requirement. The 'No Card No Start' law applies to this position - a Blue Card must be obtained prior to commencement.
