Disability Support Worker -Preston VIC (Complex Needs and Community Access)
We are looking for experienced Disability Support Workers to support participants with high support needs. Depending on participant needs, your responsibilities may include:
- Providing support in home and community settings
- Supporting participants with complex behaviours and behaviours of concern
- Implementing Behaviour Support Plans (BSPs) and established support strategies
- Supporting participants with psychosocial disabilities, Autism, intellectual disabilities and complex presentations
- Assisting with personal care, daily living tasks and household routines
- Supporting community access, appointments, shopping and social activities
- Providing high-intensity supports where appropriately trained and qualified
- Medication assistance in accordance with care plans and organisational procedures
- Completing accurate progress notes, incident reports and behaviour documentation
- Recognising and appropriately responding to changes in behaviour, wellbeing or risk
- Working collaboratively with families, support coordinators, clinicians, behaviour support practitioners and other stakeholders
About You
We are looking for someone who:
- Have previous experience supporting people with complex disabilities
- Are confident managing challenging behaviours and escalations
- Can remain calm, qualified and responsive in high-pressure situations
- Understand positive behaviour support, restrictive practices and participant rights
- Have strong communication, observation and documentation skills
- Can follow detailed care plans, risk assessments and behaviour support strategies
- Are reliable, punctual and committed to providing consistent support
- Understand professional boundaries and the importance of participant choice, dignity and independence
- Are comfortable working independently as well as within a multidisciplinary care team
Requirements
Applicants should hold or be willing to obtain relevant requirements, including:
- Certificate III/IV in Disability, Individual Support, Community Services or a related qualification
- NDIS Worker Screening Check
- Working with Children Check
- Current First Aid and CPR
- Valid Australian driver's licence
- Reliable and insured vehicle
- Current National Police Check
- Medication competency/training
- Manual Handling training
- Relevant worker training modules
