IUIH is recruiting a Social Health Care Coordinator to support Aboriginal and Torres Strait Islander clients with complex social and mental health needs. The role is based in the Goodna clinic and collaborates with Counsellors, Psychologists and other Social Health team members to ensure timely access to services and optimal client outcomes.
You will manage referrals, perform triage and contribute to client journey care reviews while maintaining the Social Health Register.
