The Chance:

Are you a dynamic and experienced Clinical Care Coordinator (CCC) looking to make a real impact? This is your chance to step into a pivotal leadership role at one of our vibrant Residential Aged Care Homes.
n
Working alongside the Residential Care Manager, you'll help lead the clinical operations of the home, ensuring our residents receive exceptional care in line with the Aged Care Quality Standards.
n
This is an ideal role for someone with strong change management skills and a genuine passion for uplifting people, culture, and care outcomes.
n
Full-time, permanent role – may include occasional weekend work
n
Based within our Residential Aged Care Home
n
To be considered, you'll need current AHPRA registration and proven clinical experience within the aged care sector.

Role & Responsibilities:
n
n
- Organising and facilitating case conferences to partner with consumers and their representatives in developing care plans;
n
- Be a clinical care leader and primary contact within the home;
n
- Provide expert case management alongside the Residential Care Manager;
n
- Monitor and improve clinical outcomes through audits and quality initiatives;
n
- Support policy development and implementation for resident care;
n
- Lead, coach, and inspire staff to deliver outstanding care.
n

About you?
n
n
- Registered Nurse with current AHPRA registration;
n
- Proven clinical and leadership experience in residential aged care;
n
- Strong skills in assessments, care planning, and change management;
n
- Sound understanding of aged care funding models;
n
- Commitment to best practice and continuous improvement.
n
