About the Role
n
Reporting to the General Manager, you will provide clinical leadership across our community nursing and aged care services.
You will oversee the delivery of high-quality clinical care, support our nursing team, manage client assessments, and ensure services are delivered in line with best practice standards.
n
This role combines clinical oversight, care management, leadership, education, and stakeholder engagement.
n
Your responsibilities will include:
n
n
- Providing clinical leadership, support, and mentoring to Registered Nurses, Care Managers, and care teams.
n
- Managing a portfolio of clients and ensuring care needs are assessed, planned, implemented, and reviewed.
n
- Conducting comprehensive clinical assessments and developing personalised care plans.
n
- Acting as a clinical resource and escalation point for complex care needs.
n
- Leading advanced wound care services, including complex wound assessments, treatment planning, and clinical reviews.
n
- Providing education and coaching to nursing staff to enhance clinical capability.
n
- Supporting care planning across:
n
- Wound care
n
- Continence management
n
- Diabetes management
n
- Medication support
n
- Coordinating nursing referrals, assessments, and clinical service delivery.
n
- Reviewing clinical documentation and ensuring compliance with regulatory requirements.
n
- Managing clinical incidents, investigations, and improvement actions.
n
- Building strong relationships with clients, families, healthcare providers, and community stakeholders.
n
- Working collaboratively with internal teams to support quality outcomes and service growth.
n
- Participating in clinical escalation support as required.
n
n
About You
n
You are an experienced Registered Nurse who is passionate about community care, clinical excellence, and supporting others to deliver exceptional outcomes.
n
You will bring:
n
n
- Current AHPRA registration as a Registered Nurse.
n
- Demonstrated leadership and people management experience.
n
- Experience within community nursing, aged care, disability, or acute care environments.
n
- Strong clinical assessment, documentation, and care planning skills.
n
- Advanced wound care knowledge and experience managing complex wounds.
n
- Experience mentoring, educating, and supporting other clinicians.
n
- Strong understanding of aged care, Home Care Packages, and community-based service delivery.
n
- Excellent communication, organisation, and stakeholder engagement skills.
n
- Ability to work autonomously while contributing positively to a collaborative team environment.
n
- Current Police Check, First Aid Certificate, CPR certification, and NDIS Worker Screening Check (or willingness to obtain).
n
- Current Australian Driver's Licence and reliable vehicle.
n
