n Transition Care Program – Commencing 2026 (Casual Roles)

We are seeking qualified Enrolled Nurses (ENs) and Personal Care Workers (PCWs) to join our new Transition Care Program, commencing 2026, on a casual employment basis.

n

About the Transition Care Program

n

The Transitional Care Program provides goal-oriented, time-limited, and focused care to support older people following a hospital stay. The program aims to help clients regain independence and return home, rather than prematurely entering residential aged care.

n

Services include:

n

n
- Nursing support
n
- All services are designed to maintain and improve physical and cognitive functioning.
n

n

The program supports:

n

n
- Adults aged 65 years and over
n
- Aboriginal and Torres Strait Islander clients aged 50 years and over
n
- Clients with an age-related illness
n

n

For Personal Care Workers:

n

n
- Certificate III in Aged Care, Disability, or Individual Support
n

n

For Enrolled Nurses:

n

n




- Current AHPRA registration as an Enrolled Nurse
n

n

For All Applicants:

n

n
- NDIS Worker Screening Check (NDIS Workers Screen Card)
n
- NDIS Orientation Module Certificate
n
- Current First Aid & CPR
n
- National Criminal Record Check (Aged Care)
n
- Current mandatory vaccinations apply (NSW Health -Covid -19, Hepatitis B, Influenza)
n
- Current driver's licence with comprehensive insurance
n
- Reliable vehicle
n
- Ability to work within a multidisciplinary, client-centred care model
n
