At Jewish Care Victoria, you'll be part of our Active Living Services team supporting older adults through a person-centred, evidence-based approach that prioritises continuity of care and long-term outcomes.

n

This role offers variety, autonomy and the chance to truly connect with your clients - helping them stay active and independent in the community they love.

n

What you will do:

n

n
- Initial assessment of clients and development of individualised care plans
n
- Perform client reviews using appropriate outcome measures
n
- Promote independence and functional abilities for both residential and community clients, with a strong focus on health promotion
n
- Communicate client outcomes at relevant clinical/team meetings and documentation in the client data base
n
- Promotion and understanding of cultural diversity in care services
n

n

You will have:

n

n
- Degree in Occupational Therapy
n
- Registration with the Australian Health Practitioners Regulation Agency (AHPRA)
n
- Demonstrated knowledge of a Wellness Reablement approach to service delivery in community aged care
n
- Experience in the development, facilitation and evaluation of group programs is desirable, but not essential
n
- Proven clinical experience in community aged care or a related field
n
