Pacific Coast Post-Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our exceptional in-house rehabilitation team in Salinas, California.

Key Responsibilities

n

n
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.
n
- Provide skilled physical therapy interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re‐education.
n
- Monitor resident progress and adjust treatment plans to maximize functional outcomes.
n
- Guide residents through their rehabilitation journey from acute recovery toward safe discharge and community reintegration.
n
- Collaborate with interdisciplinary team members to coordinate effective, resident‐centered care.
n
- Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.
n
- Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
n




- Participate in care conferences, clinical meetings, and quality improvement initiatives.

Qualifications

- Current California Physical Therapist (PT) license in valuable standing.
n
- Strong clinical reasoning, communication, and teamwork skills.
n
- Passion for geriatric, sub‐acute, and functional rehabilitation.
n
- Commitment to delivering evidence‐based, patient‐centered care.
n
