About the Role
n
An exciting opportunity exists for an experienced Grade 2 Social Worker to join our team on a permanent part-time basis.
n
Working within a supportive multidisciplinary environment, you will provide high-quality psychosocial assessment, intervention and discharge planning for patients within a sub-acute healthcare setting. This role offers the opportunity to work collaboratively with patients, families, carers and healthcare professionals to achieve positive patient outcomes and support safe transitions of care.
n
We are seeking a clinician with demonstrated experience in hospital social work who is passionate about person‐centred care and committed to delivering evidence‐based practice.
n
Experience in a sub-acute setting, particularly within Geriatric Evaluation and Management (GEM), will be highly regarded.
n
Classification: Grade 2 Social Worker (SC21 – SC24)
n Salary: $90,235 – $103,724 per annum, pro rata
n
Key Responsibilities
n
n
- Undertake comprehensive psychosocial assessments and formulate appropriate intervention and discharge plans.
n
- Apply advanced clinical reasoning to support patients with complex psychosocial needs.
n
- Liaise effectively with patients, families, carers and multidisciplinary team members to coordinate care and facilitate positive outcomes.
n
- Support safe discharge planning and continuity of care.
n
- Participate in quality improvement, service development and research activities.
n
- Provide supervision and contribute to student and clinical education programs as required.
n
- Foster strong working relationships within the department,
across the organisation and with community‐based services and stakeholders.

Your Contribution
n
n
- A Bachelor Degree or Qualifying Master's Degree in Social Work.
n
- Significant social work experience within a hospital or related inpatient healthcare setting.
n
- Demonstrated ability to undertake complex psychosocial assessments and interventions.
n
- Strong communication, interpersonal and relationship‐building skills.
n
- Experience working collaboratively within multidisciplinary teams.
n
- Current National Criminal History Check, or willingness to obtain.
n
- Experience in sub‐acute care and/or Geriatric Evaluation and Management (GEM) services will be highly regarded.
