- Assessing functional care needs of older adults living at home

- Setting care goals and providing treatment and intervention

- Helping clients stay active, independent, and at home
n
- Performing home visits or running group activities for clients accessed Support at Home or CHSP services
n
- Delivering person-centred care in line with HammondCare and AHPRA values
n

Requirements

n

n
- Tertiary qualified Physiotherapist with current AHPRA registration
n
- Practical experience or sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting
n
- Strong communicator
n
- Ability to plan and manage time effectively
n
- Enjoy working independently as part of a multidisciplinary team
n
- Access to a reliable vehicle and valid driver's license with a safe driving record
n
