Primary objective

Conduct clinical and non-clinical assessments for aged clients referred through My Aged Care Portal using the Integrated

Assessment Tool. Provision of Care coordination for clients on Transition Care Program (TCP). Deliver high quality, culturally appropriate and effective allied health services for Commonwealth Home Support Services (CHSP).





Context statement The Aged Care Assessment Service is a Commonwealth funded program which has legislated delegation authority to approve clients for Residential Aged Care, Respite Care, Support at Home, TCP and CHSP.

Key duties and responsibilities

1. Undertake aged care assessments, consultations, education and interventions for aged people across the Top End.
2. Provide integrated case management that enables TCP care recipients to meet their goals.
3. Provide allied health services for those eligible CHSP clients across the Top End.
4. Ensure effective continuity of care through case coordination of clients with complex needs, working collaboratively with the

multidisciplinary team and with a range of aged care stakeholders.
- Ensure that a high quality service is achieved and professional clinical skills maintained and developed by participating in clinical supervision, performance management and relevant continuing education.

1. Comply with the Department of Health’s and Commonwealth Aged Care Assessment Service and Transition Care Program

guidelines and systems as required.
- Undertake supervision of undergraduates as required.
- Follow defined service quality standards,



work health and safety policies and procedures relating to the work being undertaken in order to ensure high quality, safe services and workplaces.

Selection criteria

Essential

1. Bachelor degree in Occupational Therapy or Physiotherapy and registered with the relevant Board of Australia.
2. Demonstrated post-graduate experience in providing clinical and holistic assessments and interventions to the aged

population.
- Demonstrated experience working effectively within a health multidisciplinary team, including the ability to prioritise and manage caseloads in an effective and timely manner.
- Demonstrated high quality oral and written communication skills as well as computer skills, report writing and maintaining clinical records.

1. Proven skills in problem solving, negotiating, conflict resolution, which reflect and value client participation and community

inclusiveness.
- Holds a current Northern Territory drivers licence or the ability to obtain.

2. Experience in the use of the My Aged Care assessor portal
3. Ability to travel within the Northern Territory
