About the Role

Partnership Builds is seeking a compassionate and dedicated Aged or Disabled Carer to provide person-centred support to participants in their daily activities, personal care and community participation. The role involves promoting independence, wellbeing and quality of life while delivering respectful, safe and individualised care in accordance with each participant’s needs and support plan.

Key Responsibilities

· Provide person-centred care and support to participants with disability in accordance with their individual support plans.

· Assist participants with personal hygiene, dressing, grooming, mobility and other daily living activities.

· Support participants with meal preparation, household tasks, shopping and other routine activities.

· Accompany and assist participants to access community,



social and recreational activities and promote independence.

· Provide companionship, emotional support and encouragement while respecting each participant’s dignity, choice and privacy.

· Assist participants to develop and maintain daily living skills and participate safely in their community.

· Monitor participants’ wellbeing and report changes, incidents or concerns to the appropriate supervisor.

· Maintain accurate records of care and support provided and comply with organisational policies, health and safety requirements and relevant care standards.

Required Skills and Qualifications:

- At least a relevant AQF Certificate III or higher qualification or equivalent (minimum course duration of 6 months) or 2 years of relevant work experience (At least 20 hours per week) to substitute for the qualification.
- Experience: 12 months of work experience with at least 20 hours per week relevant work experience in disability support, aged care or similar roles preferred.
- Valid Driver's License, First Aid and CPR certification, police check and Working with Children Check / DHS – DCSI – Disability Employment Screening Clearance / NDIS Worker Check or willing to complete when requested.
- Australian Citizen or Permanent Resident are encouraged to apply
