About the role

As a Pharmacotherapy Coordinator, you will provide care coordination, health education and service navigation for people accessing pharmacotherapy and primary healthcare services. You'll work collaboratively with clinicians and support workers to deliver holistic, consumer-centred care

Your duties will include

- Deliver high quality AOD administrative services, collaborating with the team to facilitate care planning for clients
- Provide care coordination to support consumers to achieve their goals, including liaising with external providers
- Work with Client Services, Intake and other relevant employees to facilitate accessible, timely and consumer-centred appointment booking
- Review GP appointment book daily to ensure appropriate management of appointments for clients and GPs

- Certificate IV or higher qualification in alcohol and other drugs, or related fields
- Completion of, or willingness and ability to complete, recognised training and competency assessment in pathology collection, specimen handling and basic wound care
- Current first aid and CPR certification, or willingness to obtain these before commencing relevant duties
- Demonstrated understanding in the delivery of care for clients with AOD, chronic disease and/or complex needs based on best practice guidelines

Appointments are subject to satisfactory completion of a Nationally Coordinated Criminal History Check, a Working with Children Check and a Pre Employment Declaration Form, together with evidence of the right to work in Australia and any required vaccinations.

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
