Purpose for this role
ThePrimary Care Developmentteam leads COORDINARE in developing and implementing relevant strategies and targeted stakeholder engagement plans.The teamis responsible forworking with stakeholders in innovative ways tofacilitateand integrate service delivery to improve the patients’ journey through the health system and improve health outcomes. The teamis responsible forthe provision of practice support, systemredesignand other health system improvement projects, pathway optimisation, coordination of care, stakeholderalliancesand communications.
The Health Coordination Consultants are highly skilled in relationship management and are responsible fordeveloping meaningful relationships and engaging with key stakeholders including, in particular, withgeneral practitioners (GPs)
in their local area.In doing so, this role is the relationship manager and change agent supporting the implementation of strategies and initiatives aimed at supporting and/or influencing stakeholders to make quality and system improvements for better health outcomes in our region.
Whilst it is important that the role operate as part of the Primary Care Development Team and implement identified strategies consistently, it is expected that they will be working autonomously for the majority of their time.The ability to exercise independent judgment, identify and progress local priorities and demonstrate performance through the achievement of identified outcomes is critical.This approach needs to be balanced with seeking out opportunities to collaborate, ensuring open lines of communication are maintained, and issues are escalated as required.
Skills and Experience
- Experience working in health, community services, stakeholder engagement, service improvement, or a related field.
- Strong relationship-building and communication skills, with the ability to influence and engage a range of stakeholders.
- Experience coordinating projects, programs, or improvement initiatives.
- The ability to analyse information and use data to identify opportunities and support decision-making.
- Robust organisational skills and the ability to manage competing priorities.
- Confidence working independently while remaining connected and collaborative with a broader team.
- Knowledge of the Australian health system, primary care sector, or related services.
Tertiary qualifications in health, healthcare, public health, health administration, community services, or a related discipline; or an equivalent combination of relevant skills, knowledge and experience.
- Working hours: Permanent part-time role, 3 days per week (0.6 FTE).
- Remuneration: $103,499 per annum (pro-rata for part time), plus superannuation and other benefits.
- Office Location: Fyshwick (supporting the Queanbeyan Palerang General Practice Cluster).
