Job Description
This is an exciting opportunity for an experienced Community Mental Health Clinician (4+ years) to join a leading public mental health service in Regional Victoria, delivering evidence‐based, recovery‐focused care across a range of specialised community programs. You will play a key role in supporting consumers experiencing complex mental illness by providing comprehensive assessment, crisis response, therapeutic interventions and sustained case management in a trauma‐informed, person‐centred framework.

Your Clinical Impact As a senior community clinician, you will use advanced mental health nursing or allied health skills to:
n
n
- Provide high‐quality psychiatric assessment, formulation and risk stratification, including suicide and violence risk.
n
- Lead recovery‐oriented case management, supporting consumers with complex biopsychosocial needs.
n
- Deliver structured therapeutic interventions such as CBT‐based strategies, safety planning, relapse prevention, sensory modulation, psychoeducation and family‐inclusive practice.
n
- Conduct crisis assessments in the community, ED, home‐based care or telehealth environments.
n
- Undertake Triage & Intake assessments using clinical prioritisation frameworks and assertive follow‐up.
n
- Partner with HOPE or post‐discharge teams to provide assertive outreach, engagement and ongoing safety planning.
n
- Collaborate with GPs, psychiatrists, AOD services, NDIS providers, housing, and other community agencies to ensure integrated care.
n
- Drive clinical governance through participation in clinical reviews, reflective practice, care planning meetings and multidisciplinary case conferences.
n
n
Key Responsibilities:
n
n
- Conduct comprehensive mental health assessments including MSE, risk formulation, diagnostic considerations and needs assessment.
n
- Develop and implement individualised treatment, support and recovery plans.
n
- Provide crisis intervention, including short‐term intensive support and escalation where required.
n
- Deliver evidence‐based therapeutic interventions, both individual and group‐based.
n
- Coordinate care across multiple service providers and promote multi‐agency collaboration.
n
- Maintain high‐quality clinical documentation aligned with the Mental Health & Wellbeing Act 2022.
n
- Participate actively in multidisciplinary team meetings, clinical governance, and ongoing service development.
n
- Contribute to quality improvement, service innovation and best‐practice care pathways.
n
n
Key Requirements:
n
n
- Minimum 2+ years recent experience in Community Mental Health (public mental health preferred).
n
- Tertiary qualification in Nursing, Social Work, Psychology or Occupational Therapy with current AHPRA or AASW registration.
n
- Demonstrated expertise in:
n
- Complex risk assessment & crisis response
n
- Care coordination & recovery‐oriented practice
n
- Trauma‐informed, culturally safe and consumer‐led care
n
- Strong understanding of the Mental Health & Wellbeing Act 2022, recovery model, and Victorian public mental health frameworks.
n
- Ability to work autonomously and collaboratively in a multidisciplinary environment.
n
- Excellent interpersonal, written and verbal communication skills.
n
- Valid driver's licence (community work required).
