The Disability Support Worker – Team Leader (TL) is responsible for providing operational leadership and oversight of service delivery for allocated participants at LAST. The role ensures supports are delivered safely, consistently, and in alignment with individual goals, NDIS Practice Standards, and organisational requirements. The Team Leader supervises, mentors, and supports Disability Support Workers and Key Workers, manages incidents and risks, and acts as the central point of coordination between participants, families, staff, and external stakeholders.

Key responsibilities

- Ensure all service delivery aligns with NDIS Practice Standards, organisational policies, and safeguarding requirements.
- Provide operational leadership for allocated Clients, ensuring service delivery is safe, person-centred, responsive and aligned with individual needs, goals and preferences.
- Act as the primary point of contact for allocated Clients, their families, support staff and external stakeholders in relation to day-to-day care, service delivery and operational matters.
- Oversee the onboarding of recent Participants, ensuring supports commence safely, accurately and in accordance with organisational procedures.
- Supervise, mentor and coach Disability Support Workers and Key Workers to build capability, improve practice and support high‑quality service delivery.
- Oversee and manage Client‑related reports, including incidents recorded within the Brevity Incident Management System, ensuring follow‑up actions, investigations and reporting are completed within required timeframes.
- Identify trends, risks and improvement opportunities arising from incidents, feedback and reports, and contribute these to the organisation's Continuous Improvement Plan.
- Escalate restrictive practice concerns, unauthorised restrictive practices or significant risks to the Nurse Managing Director in accordance with policy and legislative requirements.
- Support the Compliance Officer by reviewing Client documentation,



providing required information and completing Client‑specific actions to ensure Brevity Client Checklists remain current as items and actions become due.
- Ensure organisational policies, procedures and work instructions are understood and applied by support staff in day‑to‑day practice.

About you

- Certificate IV or higher in Human Services, Disability, Community Services or a related discipline.
- Demonstrated experience in a Team Leader, Senior Disability Support Worker or similar role within a disability or human services environment.
- Demonstrated experience supporting the delivery of services to people with disability, including oversight of support staff, incident response and stakeholder engagement.
- Sound understanding of the NDIS Practice Standards, NDIS Code of Conduct and safeguarding requirements.
- Demonstrated knowledge of PBSPs, restrictive practices and the Team Leader's role in overseeing implementation and escalation.
- Current HLTAID011 Provide First Aid Certification.
- Current HLTAID009 Provide Cardiopulmonary Resuscitation Certification.
- Current HLTHPS006 Assist Clients with Medication Certification (or equivalent).
- Demonstrated leadership capability in a disability or human services environment, including the ability to supervise, mentor and coach Disability Support Workers and Key Workers.
- Strong operational judgement and decision‑making skills to manage incidents, risks and service delivery issues within policy and legislative requirements.
