About the opportunity

IFYS has an exciting opportunity for a Housing Case Manager to join our Accommodation Services team. This role forms part of a new homelessness outreach initiative focused on supporting people experiencing rough sleeping and housing instability to access safe, sustainable housing outcomes.

The Housing Case Manager provides assertive outreach and case management support to people experiencing homelessness, using a Housing First and trauma-informed approach. Working across public and community settings, the role builds pathways into accommodation and housing while connecting clients with the services and supports needed to address barriers to long‑term stability.

- 1x fixed-term part‑time position (60 hours per fortnight); 2 x fixed‑term full‑time positions contracted until 31/12/2027
- Employment based in Nambour
- Working Monday to Friday, roster outlined below

Responsibilities of the role

- Deliver case management services with a focus on achieving sustainable housing outcomes
- Conduct assertive outreach to individuals sleeping rough and those experiencing housing instability in public and community settings
- Complete comprehensive assessments, identify barriers to housing, and develop individualised support plans using a Housing First framework
- Assist clients to access short‑term, and long‑term housing options
- Facilitate referrals and connections to specialist services to address health, wellbeing, financial, and social support needs
- Build and maintain effective relationships with key stakeholders to improve client outcomes
- Maintain accurate client records, assessments, referrals, and case notes using QHIP and SHIP systems
- Monitor emerging trends, housing needs, and service demand, providing feedback and reporting
- Operate in accordance with safe work practices, risk assessments, and organisational procedures

Requirements for the role

- Demonstrated experience delivering case management services to people experiencing homelessness or individuals with complex needs
- Sound understanding of Housing First principles, trauma‑informed practice, and strengths‑based approaches
- Experience conducting client assessments, risk identification and management and support planning
- Well‑developed communication and relationship‑building skills with the ability to engage people from diverse backgrounds
- Strong stakeholder engagement and referral networking skills




- a high level of organisational and administrative skills, including accurate record keeping and case documentation
- Ability to prioritise competing demands in a dynamic outreach environment
- A Certificate IV or higher qualification in Community Services, Human Services, Social Work, or a related discipline is highly regarded

Must hold or be willing to obtain if successful

- Valid Queensland Driver’s License with no restrictions
- QLD Working with Children Check (Paid Blue Card)
- Licensed Care Suitability Check (LCS2)
- National Crime Check (anew check will be completed through IFYS systems)
- Ability to provide two recent line manager references including your current employer

Fixed full‑time roles

- Monday to Friday, 9am to 5pm

Fixed part‑time role

- Monday to Friday, 8am – 4pm
