About the Role:

Echuca Regional Health is seeking an experienced and compassionate Social Worker to join our Community Rehabilitation Program (CRP). This is an exciting chance to work within a multidisciplinary team, supporting clients to achieve their rehabilitation goals and maintain independence within their community.

As a key member of the Community Rehabilitation team, you will provide person-centred assessment, care planning, counselling, education, advocacy, and service coordination to clients and their families.

Key responsibilities include:

- Conduct comprehensive psychosocial assessments and develop individualised care plans.
- Provide counselling, support, and education to clients and their families.
- Facilitate access to appropriate community services and support networks.
- Work collaboratively with multidisciplinary health professionals to coordinate care.
- Support clients through rehabilitation and transition to community-based services.
- Assist with referral management, program development, and data collection activities.
- Contribute to a positive, collaborative team culture that reflects ERH's CARE Values.

Qualifications/experience required:

- Qualification in Social Work and eligibility for professional membership.
- Minimum five years' clinical experience,



including demonstrated knowledge and skills in rehabilitation.
- Strong assessment, care planning, and case management skills.

This position requires completion of a satisfactory National Police Check and (where relevant) provision of a Working with Children Check and check on the Disability Worker Exclusion Scheme prior to appointment. Vaccination status in line with policy and procedure at this time. Applicants must hold Australian Citizenship or hold Australian Work Rights to be eligible. Financial support towards VISAS is not available.
