Job Description
Doorway Nest Pty Ltd SDA Community Worker & Participant Wellbeing Coordinator Doorway Nest Pty Ltd trading as ILS Homes is seeking a compassionate and experienced SDA Community Worker & Participant Wellbeing Coordinator to join our team. This role provides an exciting opportunity to support NDIS participants to access and thrive within Specialist Disability Accommodation (SDA). You will build meaningful relationships with participants, understand their individual goals and support needs, and help ensure they experience protected, person-centred and high-quality accommodation.
Working closely with participants, families, Support Coordinators, allied health professionals, service providers and community organisations, you will promote participant choice, independence, dignity, inclusion and wellbeing. Key Responsibilities Participant Engagement, Intake & SDA Placement Build positive and respectful relationships with participants and their support networks. Meet with participants to understand their goals, preferences, lifestyle, cultural needs and accommodation aspirations.
Conduct participant intake assessments and gather relevant documentation. Assess participant support needs, functional capacity, accessibility requirements and behavioural considerations. Assist participants to identify suitable SDA accommodation options aligned with their individual needs and preferences.
Coordinate participant onboarding, orientation and transition into SDA accommodation. Maintain accurate participant records, assessments and progress documentation.
Participant Support & Wellbeing Regularly engage with participants to monitor their wellbeing, satisfaction and changing support needs. Encourage participant choice, control and independence. Support participants to work towards their personal goals and maximise independence within their home environment. Advocate for participant rights and ensure participants are treated with dignity, respect and compassion.
Work collaboratively with participants to resolve concerns and improve their accommodation experience. Promote social inclusion and connection with families, communities and support networks.
Community & Stakeholder Engagement Develop professional relationships with Support Coordinators, Support Providers, Local Area Coordinators, hospitals, rehabilitation services, allied health professionals, families and community organisations. Assist participants to connect with appropriate services and supports. Participate in community networking activities to strengthen referral pathways.
Support participant access to suitable SDA accommodation through professional and community networks. Participant Safety & Safeguarding Conduct participant-centred risk assessments before and during accommodation placements. Identify health, safety, behavioural and environmental risks. Work collaboratively with participants and stakeholders to develop appropriate risk management strategies. Monitor participant wellbeing and respond to emerging concerns. Escalate significant risks in accordance with organisational requirements. Promote safeguarding practices that protect participants from abuse, neglect, exploitation, discrimination and unauthorised restrictive practices.
Advocacy, Feedback & Quality Promote and uphold participant rights. Support participants to make informed decisions regarding their accommodation and supports. Advocate for participant interests when engaging with families, providers and other stakeholders. Encourage and respond appropriately to participant feedback. Receive and respond to participant complaints respectfully and professionally. Respond appropriately to participant incidents and assist with follow-up actions. Maintain accurate incident, complaint and participant documentation.
Contribute to continuous quality improvement initiatives.
NDIS Compliance Maintain accurate participant assessments, case notes and service documentation. Support compliance with the NDIS Practice Standards, NDIS Code of Conduct and organisational policies. Contribute to internal quality reviews and continuous improvement activities. Assist with audit preparation and supporting documentation when required. Promote person-centred and high-quality practices across participant services.
Essential Qualifications & Experience
Diploma of Community Services or higher qualification in Community Services, Disability, Social Work, Human Services or a related discipline. Minimum one year of experience in disability services, community services, SDA, SIL, aged care, health or a related sector. Experience supporting NDIS participants and working with their support networks. Experience conducting participant assessments, support planning and case management. Experience identifying and managing participant risks. Experience managing participant incidents, complaints and safeguarding matters. Understanding of the NDIS Practice Standards and person-centred service delivery.
