As the Care Manager Team Leader, you will support and supervise Care Managers to deliver high-quality, relationship-centred community-based services. Reporting to the Business Operations Manager, you will help ensure care management practice is consistent,compliantand aligned with legislative,regulatoryand organisational requirements.

Key Responsibilities

Support consistent delivery of high-quality, relationship-centred services across the client journey.

Provide day-to-day supervision and guidance to Care Managers across care planning, documentation, riskmonitoringand service coordination.

Act as the first point of escalation for complex client matters, safeguardingconcernsand service risks.

Conduct regular audits of care plans, client reviews, budgets,



riskassessmentsand documentation tomaintaincompliance and audit readiness.

Lead capability development through onboarding, mentoring, performancesupportand targeted education for Care Managers.

About you

We’re looking for a people-focused leader who enjoys developing others and isn’t afraid to roll up their sleeves. You’ll bring:

Experience leading or supporting teams in aged care, community services or a similar environment.

Solid people leadership, with the ability to coach, mentor and support teams through change.

A practical understanding of care management, quality, compliance, risk and safeguarding. SIRS knowledge would be highly regarded.

Strong organisation and problem-solving skills, with confidence managing competing priorities and complex client matters.

A collaborative, resilient and values-led approach, with a genuine commitment to clients, families and your team.

A tertiary qualification in Nursing, Allied Health, Social Work or a related discipline will be highly regarded.
