Specialising in High Intensity Medical Supports or Youth and Adolescent Support
Full time | Newcastle and Hunter Region
Activated Concepts is growing. As our services expand and our work with DCJ continues to grow, we are looking to bring additional specialist leadership into our Disability Support Services team, alongside our existing Team Leader.
We want this appointment to bring a genuine point of difference from our current Team Leader, either in high intensity medical support or in youth and adolescent support. Depending on the calibre of applicants, this may result in one appointment or two.
This is not a caseload made up solely of high intensity medical participants or solely of young people. You will work across our broader disability support caseload, which spans a wide range of needs, and bring your specialist skill and interest in one of these two areas to that work.

Activated Concepts is a community based provider delivering trauma informed services across mental health, allied health, disability, family support, aged care support under the Commonwealth Home Support Programme, nursing, behaviour support and training.

The Role
You will lead a broader disability support caseload, bringing hands on leadership across the team while applying your specialist focus, either clinical oversight for participants with high intensity medical needs, working closely with GPs, nurses and allied health professionals, or trauma-informed leadership for young people and families, working closely with DCJ, schools and families.
Key Skills & Experience, High Intensity Medical Supports
- Enrolled Nurse, Diploma of Nursing qualified and currently registered, with experience supporting people with complex health and medical needs in community and disability settings
- Experience supporting and supervising complex medical care, including PEG management, catheter care and other clinical supports, ensuring care is delivered safely and in accordance with individual care plans, clinical guidance and organisational procedures
- Confident interpreting and implementing clinical and individualised care plans, identifying changes in a participant's health, presentation or support needs and appropriately escalating concerns to relevant health professionals and stakeholders
- Clinical leadership and staff supervision, providing guidance and oversight to support workers delivering complex supports, promoting safe practice, competency and adherence to participant-specific care plans
- Strong multidisciplinary collaboration, working closely with GPs, nurses, allied health professionals and other stakeholders to ensure supports remain responsive to changing participant needs
Key Skills & Experience, Youth and Adolescent Support
- Passionate about trauma-informed, culturally safe and person-centred practice, with a strong commitment to supporting people to build capacity, achieve their goals and experience meaningful, lasting change
- Experience in Out of Home Care and Youth Work, supporting young people with complex trauma, challenging behaviours and diverse support needs through strengths-based and therapeutic approaches
- Strong team-oriented practice, working alongside DCJ, schools, families, health professionals, allied health and other external agencies to develop and implement coordinated support strategies
- Experience overseeing and implementing risk management strategies, including Risk Assessments, Behaviour Support Plans, restrictive practices, Safety Plans and other therapeutic or clinical interventions
- Experienced in responding to critical incidents, providing calm and effective operational leadership while ensuring participant safety, appropriate escalation and follow-up actions
Both Specialisations
- Strong documentation and compliance skills, including reviewing incident reports, progress notes, care documentation and other records to support quality service delivery and legislative and regulatory compliance
- Committed to building staff capability and confidence, providing practical guidance, mentoring and clinical oversight to support workers
Reporting and Structure
- You will report directly to the Executive Manager, Family, Disability and Community Support Services
- You will work alongside our existing Team Leader and leadership team, bringing your specialisation into the mix
- You will provide direct leadership, supervision and clinical guidance to your team
About You
We're looking for someone who leads with warmth,
confidence and a calm head, especially given the complexity of some of the people we support. You don't need years of formal staff management experience to apply. We warmly welcome senior Support Workers and senior practitioners who are ready to grow into their first team leadership role. We'll support your development along the way, including working towards a leadership or management qualification as part of your growth in the role.

Classification and Conditions
- This role is classified within the SCHADS Award at Level 3, with the specific pay point (3.1 to 3.4) set according to the successful candidate's skills, qualifications and experience
- This is a newly created position. Your specialised portion of the caseload is still developing as we grow into this area, and pay and classification are set in line with our existing Team Leader position regardless of current caseload size
- Full time, based in Newcastle and the Hunter
- RDO, work from home and flexible work options may be considered in line with role requirements, performance and business needs

Requirements
Applicants applying under the High Intensity Medical Supports specialisation must hold, or be willing to obtain, current AHPRA registration as an Enrolled Nurse.

- Other role specific compliance checks or training
