Right at Home Gosford are seeking a qualified and experienced ClinicalCare Partner - EN/RN to help our clients live empowered and independent lives at home. Our mission is to improve the quality of life for those we serve.

Your new role will be responsible for:

n

Meeting with clients and their families and coordinating their needs within their funding model

n

Conducting client care assessments and creating care plans to best meet client requirements

n

Handling information requests, feedback,



and general enquiries

n

Allocating and training care workers to match client needs

n

Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs

n

Performing some community nursing care as needed

n

To be successful in this role, you will need:

n

n
- A current AHPRA Registration
n
- Qualifications and experience in Nursing
n
- Highly developed verbal and written communication skills
n
- Strong organisational, problem solving and time-management skills
n
- Current Australian National Police Check & Working with Children Check
n
- Current CPR certificate
n
- Full Driver's license and your own reliable, insured vehicle
n
- The right to work in Australia
n
- Sound understanding of the Aged Care and Disability government funding systems
n
- Demonstrated experience supporting staff
n
