About the Role

As a Case Manager, you'll:
n

- Conduct comprehensive assessments to understand each client's unique needs.
n
- Collaborate with healthcare professionals and service providers to deliver tailored support.
n
- Build strong relationships with clients and key stakeholders.
n
- Ensure government funding is used effectively and in line with guidelines.
n
- Exceptional organisational skills and ability to self-manage.
n
- Strong communication skills – both written and verbal.
n
- Proficiency in using Microsoft Suite and client management systems.
n
- Commitment to working within a multidisciplinary team.
n
n
The Successful Applicant will have:
n
n
- Current Working with Vulnerable People/Children Check.
n
- Current National Police Check.
n
- Current Australian Driver's Licence.
n
n
