About the Role
n
Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focused environment.
n
Work Schedule
n
7‐day, 24‐hour rotating roster including day, afternoon, and night shifts. Permanent full‐time, part‐time and casual positions available.
n
Key Responsibilities
n
n
- Deliver individualised nursing care as part of a multi‐disciplinary team
n
- Liaise and effectively communicate with relevant medical, nursing and allied health team members
n
- Promote evidence‐based practice that has a problem‐solving approach
n
- Encourage patients, carers and families to participate in all stages of care
n
- When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback
n

Skills & Experience
n
n
- Registered Nurse with current AHPRA registration
n
- Excellent communication and problem‐solving skills
n
- Knowledge of contemporary nursing theory and practice in relation to the care of sub‐acute patients/clients
n
- Thorough understanding of quality systems and health services accreditation
n
n

All appointments are subject to a satisfactory police records check, immunisation clearance and Working with Children check and/or NDIS Worker Screening Check.

All employees must be willing and able to wear Personal Protective Equipment (PPE) when required, which at a minimum is a surgical mask. Additional PPE requirements are role‐dependent.
