Key Responsibilities:
n
n
- Assist participants in understanding and implementing their NDIS plans.
n
- Work collaboratively with participants to identify their goals and develop strategies to achieve them.
n
- Coordinate supports by engaging with a wide range of service providers, allied health professionals, community organisations, and networks.
n
- Collaborate with support workers to arrange schedules, manage tasks, and ensure effective service delivery.
n
- Monitor and review participant progress,
ensuring services remain aligned with their goals and changing needs.
n
- Provide participants with up-to-date information regarding their options, rights, and responsibilities under the NDIS.
n
- Maintain accurate, professional, and up-to-date records, including case notes, reports, and participant documentation.
n
- Act as the primary point of contact for participants, families, therapists, support workers, and other stakeholders, responding professionally and promptly to enquiries.
n
- Advocate for participants to ensure they receive high-quality, person-centred support.
n
- Plan, coordinate and participate in participant outings, community activities, and social inclusion programs where appropriate.
n
- Develop and maintain strong relationships with local community organisations, allied health providers, hospitals, support coordinators, and referral partners.
n
- Represent Angels of Mercy at networking events, community functions, expos, and other promotional activities to build professional relationships and support organisational growth.
n
- Contribute to continuous improvement initiatives and support the ongoing growth of the organisation.
n
n
What We're Looking For:
n
n
- Must speak Arabic.
n
- Assyrian/Chaldean language skills are highly desirable.
n
- Experience in NDIS Support Coordination, Case Management,
Community Services, or a related role.
n
- Sound understanding of the NDIS framework, policies, and procedures.
n
- Excellent written and verbal English communication skills.
n
- Excellent organisational, planning, and time-management skills.
n
- Strong attention to detail and the ability to follow through on multiple tasks accurately.
n
- Ability to build positive relationships with participants, families, therapists, support workers, and service providers.
n
- Ability to work independently, manage competing priorities, and confidently solve problems.
n
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and other relevant software.
n
- A proactive, professional, compassionate, and reliable attitude.
n
- A valid driver's licence and access to a reliable vehicle.
n
- NDIS Worker Screening Check, Working with Children Check, and Police Check (or willingness to obtain).
n
- Completed NDIS Worker Orientation Module (or willingness to complete before commencing).
n
- Must have valid work rights in Australia.
n
n
Working Hours:
n
n
- Part-time: 4 days per week, 9:30 AM to 4:00 PM.
n
n
