About the Role:

n

As a Care Coordinator, you will play a vital role in planning and managing care services for clients receiving in-home support. You e2 a0 e2 a0 e2 0 0ll work closely with our field staff, clients, families, and health professionals to deliver services that truly make a difference. We are looking for a team member to help us cover maternity leave (with potential to be an ongoing role)

Key responsibilities include:

- Conducting initial assessments and developing person-centred care plans
n
- Coordinating and scheduling care services with accuracy and efficiency
n
- Supporting carers with guidance, resources, and regular check-ins
n
- Maintaining compliance with care standards and documentation
n
- Ensuring clients feel heard, respected, and supported at all times
n

We2 a0 e2 0 e2 0 0re seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.

You will ideally have:

n

- Experience in care coordination, case management, or a similar role
n
- A background in aged care, disability support, or community services
n
- Excellent communication, time management, and problem-solving skills
n
- A strong ethical approach and attention to detail
n
- Confidence using technology and care management systems
n
- A current driver2 a0 3s licence and access to a reliable vehicle
n

Qualifications

n

- Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience
n
- Knowledge of Home Care Packages (HCP) and or NDIS (Desirable)
