About the Department / Program

The Intensive Community Treatment (ICT) Team offers home based care as an alternative to treatment in an Older Adult Mental Health (OAMHS) acute inpatient unit. Functioning is on an outreach basis, delivering acute care to the person in their home context. The team is based at the Endeavour Hills site and covers the entire Monash Health Mental Health Catchment Area which includes, Bayside, Kingston, Glen Eira, Monash, Greater Dandenong, Casey, Cardinia and part of Frankston Local Government Areas.

ICT sits within the continuum of care of the broader mental health service system. The ICT team is embedded within the Older Adult community teams such as the Aged Persons Assessment and Treatment Services (APATS), Residential Support Program and Older Adult Triage and has robust links with Older inpatient units as well as Monash run Residential Aged Care. We work closely with psychosocial and residential support providers, including local council or non-government organisations, to provide holistic care to older people.

About the Role

Join our team as a Social Worker (Grade 3) on a full-time ongoing basis to work our Older Adults Mental Health service,



within seven community teams across two regions; Middle South (based in Kingston Centre) and Dandenong (based in Endeavour Hills)

- Biala, Kingston Centre

In this role, you will:

- Provide comprehensive mental health assessments, including mental state examinations, risk assessments and crisis management for older adults experiencing acute mental health concerns.
- Deliver evidence-based psychosocial interventions, care coordination, advocacy and support to consumers, families and carers.
- Work collaboratively with multidisciplinary teams, community services and external agencies to develop and implement individualised treatment plans.
- Provide specialist Social Work knowledge and clinical expertise to support complex mental health presentations, discharge planning and access to appropriate community supports.
- Contribute to service improvement activities, professional development, supervision and supporting the growth of junior clinicians and students.

Travel between sites and community locations may be required, with hybrid working arrangements available where appropriate.

The position requires participation in a 7-day rotating roster, including morning shifts, weekend work and weekday mid shifts (11:30am–8:00pm).

About You

To be successful in this role, you will bring the following demonstrated skills, experience and attributes:

- Demonstrated experience providing comprehensive mental health assessments, psychosocial interventions and risk management for people experiencing acute and complex mental health presentations.
- Strong understanding of recovery-oriented practice, family and carer inclusive practice, and supported decision-making principles.
- Ability to work autonomously while contributing effectively within a multidisciplinary team environment.
- Excellent communication, interpersonal and problem-solving skills, with the ability to build strong relationships with consumers, families, carers and external service providers.
- Demonstrated clinical leadership skills, including supporting colleagues, students and junior clinicians through supervision,



education and knowledge sharing.
- Commitment to evidence-based practice, continuous improvement and delivering safe, high-quality consumer-centred care.

Mandatory requirements of the role:

- Bachelor of Social Work, Master of Social Work Qualifying, Master of Social Work, or equivalent qualification recognised by the Australian Association of Social Workers (AASW).
- Eligible for ordinary membership with the Australian Association of Social Workers (AASW).
- Accredited Social Worker and/or Accredited Mental Health Social Worker status as approved by the AASW.
