We are seeking a compassionate and motivated Community Services Worker to join our team. In this role, you will support individuals and families by providing person-centred services that promote independence, wellbeing, and community participation.

Key Responsibilities:

- Provide practical and emotional support to clients.
n
- Develop and implement individual support plans.
n
- Maintain accurate case notes and client records.
n
- Collaborate with families, community organisations, and other professionals.
n
- Advocate for clients and connect them with appropriate services.
n
- Ensure all work is completed in accordance with organisational policies and relevant legislation.
n

Essential Requirements:
n
n
- Certificate IV or Diploma in Community Services (or equivalent).
n
- Excellent communication and interpersonal skills.
n
- Ability to work independently and as part of a team.
n
- Current driver's licence and reliable transport.
n
- National Police Check and Working with Children Check (or willingness to obtain).
n
- First Aid and CPR certification (desirable).
n
