About the Role:

n

As a Community Clinical Manager, where you'll energise, guide, and develop your team to deliver outstanding, person-centred care across your region. You'll play a key role in shaping a connected, positive, and supportive community for every client you serve.

n

Bring your excellent communication and leadership skills to the forefront as you advise clients on their clinical needs, complete in-home assessments, and ensure care plans meet lifestyle goals, package requirements, and organisational standards - all while championing compliance and quality.

n

With your background in training and developing clinical teams, you'll lead with collaboration, identify growth opportunities, and empower your staff to excel and continuously improve.

Main Duties:

- Provide clinical guidance to participants & care staff
n
- Undertake assessments & case conferencing in line with standards & funding streams
n
- Accurate completion, review & updating of relevant documentation
n
- Motivation,



training & development of clinical team members
n
- Ensure compliance & quality indicators are met
n

Skills & Requirements:

n

n
- AHPRA registration as a Registered Nurse with leadership experience & full Australian working rights
n
- Up to date working knowledge & experience achieving accreditation standards
n
- Robust understanding of HCP, NDIS & DVA funding streams - those with residential aged care experience will also be conidered
n
