About the Role
Reporting to the Coordinator - Community Operations, this role supports older people and eligible community members to remain active, connected and independent through the delivery of social support services.
Core responsibilities include:
- Providing personal support to groups of older people during Council led community outings and activities.
- Providing manual handling support including storing or unpacking mobility aids.
- Developing and maintaining care plans,
progress notes and client care documentation and updating Customer Relationship Management System (Carelink).
- Support intake, assessment and service commencement activities.
- Monitor client wellbeing and communicate emerging needs and risks.
- Contribute to positive client outcomes through collaborative and high-quality service delivery.
About You
- Certificate III in Individual Support (Ageing), Aged Care, Home & Community Care or equivalent.
- Demonstrated experience working within the aged care or disability sector.
- Experience with client documentation, progress notes and care planning.
- Carelink experience is highly regarded.
