About the program

Jesuit Social Services Housing and Complex Needs services provide holistic support programs for people who present with multiple and complex needs, are assessed as high risk/need, have limited social and family networks, limited accommodation and support options, and are experiencing multiple and complex health problems.

Blackwood Housing and Outreach Support is funded by the Department of Families, Fairness, and Housing (DFFH) – Homes Victoria to support young people, aged 16 – 25, of all gender identities through outreach support in the community, and occupancy support through our 24/7 residential program for 8 young people for up to 12 months.

We support young people to work towards taking accountability, responsibility, choice and agency in their own lives; developing their independent living skills; engaging in education, training and employment opportunities; all with the aim of reducing reoffending, recidivism and experiencing homelessness.

About the role

We are seeking a suitable candidate to fulfil the Intensive Support Worker, Blackwood Housing and Outreach Support role. This role includes the following:

- Delivering quality support to people who present with multiple and complex needs; have been assessed as high risk and high needs; and have limited accommodation and support options




- Delivering support that reflects the social justice principles of participation; equity; access and respect
- Supporting participants to develop their independent living skills; engage in education, training and employment; with the aim of reducing the risk of reoffending and homelessness.
- There is 1 part time role (0.88 EFT) offering a rotating fortnightly roster, which includes day shifts across weekends and public holidays.
- A contract with the possibility of extension until June 2028.

Roster - Week 1:

Monday: 8am to 4:30pm - Tuesday: 7am to 3:00pm - Wednesday: OFF - Thursday: 8am to 4:30pm - Friday: OFF - Saturday: OFF - Sunday: 7am to 3pm

Roster - Week 2:

Monday: 8am to 4:30pm - Tuesday: OFF - Wednesday: 8am to 4:30pm - Thursday: 7am to 3pm - Friday: OFF - Saturday: OFF - Sunday: 8am to 4:30pm

About you

This role will suit a candidate with the following skills and experience:

1. Tertiary qualification/s and/or relevant experience in field
2. Demonstrated practice knowledge, experience and skills to case manage young people addressing multiple and complex needs,



preferably within a supported residential program and / or an outreach capacity
3. Demonstrated working knowledge and experience of homelessness and the Housing and Homelessness Service System, the criminal justice system and specialist service sectors.
4. A demonstrated commitment to delivering inclusive, safe and effective supports to young people with high support needs and potentially challenging behaviors in line with best practice principles/frameworks, including trauma informed practice, person centered practice and strengths-based practice.
5. Ability to communicate clearly and work collaboratively with multiple stakeholders across various services and disciplines.
