ACOP Credentialed Pharmacist – Aged Care | Sydney

Chemist Connect is expanding our aged-care pharmacy services and is seeking an experienced ACOP credentialed pharmacist to join our team.

We work closely with multiple aged-care facilities across Sydney , and we are looking for a pharmacist who is passionate about clinical pharmacy and wants to be embedded within residential aged-care facilities , working alongside residents, nurses, GPs and care teams.

The Opportunity

As an ACOP pharmacist with Chemist Connect, you will have the chance to:
Provide high-quality clinical pharmacy services within aged-care facilities
Conduct medication reviews and optimise medication management
Work collaboratively with GPs, nursing staff and multidisciplinary care teams
Support medication safety and quality use of medicines




Build robust relationships with residents, families and healthcare professionals
Contribute to improved health outcomes for aged-care residents
Work across different areas of Sydney , with opportunities to find locations that suit you

What We're Looking For

We are seeking a pharmacist who:
Is AHPRA registered
Holds ACOP accreditation/credentialing
Has experience or a strong interest in aged-care pharmacy
Is clinically focused and committed to improving medication outcomes
Has excellent communication and interpersonal skills
Enjoys working as part of a multidisciplinary healthcare team
Is organised, qualified and confident working independently
