Proveda is seeking a energetic Care Coordinator to help older people, people with disability and mental health needs navigate the NDIS, Home Care and related services in the Northern Sydney region.
Reporting to Practice Managers, you will deliver impartial information and guidance, support planning and service navigation, and connect clients with appropriate supports. The role is permanent part-time (4 days weekly) with versatile working after training and a cooperative team.
