About the role
Provide high‐quality mental health support within a multidisciplinary team to individuals with complex needs, including mental illness, substance use and homelessness, while working collaboratively with consumers, families,
carers and community support networks using a recovery‐focused approach
Lead, coordinate and oversee client care, ensuring services are responsive to individual consumer needs
Conduct comprehensive biopsychosocial assessments and develop, document and review appropriate treatment plans
Promote effective communication and collaboration within the multidisciplinary team and provide mental health consultation to other disciplines where appropriate
Maintain accurate clinical records and collect relevant data to support high‐quality clinical practice and continuity of care
About you
Current unrestricted NSW Drivers Licence and eligibility for membership of the Australian Association of Social Workers
Extensive experience in community mental health, working effectively within a multidisciplinary healthcare team
Demonstrated ability to deliver high‐quality, recovery‐focused care, supporting consumers, families and carers of people experiencing mental illness
Strong clinical communication and interpersonal skills, with the ability to work effectively in acute care, triage and community mental health settings
Excellent organisational, written and digital skills,
including the ability to maintain accurate documentation and confidently use electronic medical record systems
