In this role, you will: Build relationships with clients, understand their needs and goals, and act as their key contact.
Coordinate and monitor services to ensure clients receive appropriate, person-centred support.
Monitor changing needs, funding and safety, and arrange reviews or escalate concerns when required.
Maintain accurate records and ensure services meet Support at Home and organisational requirements.
Work with clients, families,



staff and providers to support positive outcomes.
