Job Description Nurse Navigator – First Nations (Identified) Emergency Department Presentation into Inpatient Care Metro South Health Logan Hospital, Meadowbrook, Queensland Temporary full-time | Six-month contract | On-site $***** per hour Indigenous Job Match is proud to advertise this identified opportunity on behalf of Metro South Health.
They are seeking an experienced Aboriginal and/or Torres Strait Islander Registered Nurse to join Logan Hospital as a First Nations Nurse Navigator.
This position will provide advanced clinical leadership, care coordination and advocacy for Aboriginal and Torres Strait Islander patients moving from an Emergency Department presentation into inpatient care.
Working across services and healthcare settings, you will help patients and families navigate complex systems, reduce barriers and access care that responds to their individual needs, goals and cultural context.
About the role As the First Nations Nurse Navigator, you will: Coordinate person-centred care across primary, secondary and tertiary healthcare settings.
Support Aboriginal and Torres Strait Islander patients throughout their healthcare journey.
Build trusted relationships with patients, families, carers, healthcare teams and community partners.
Advocate for culturally safe, connected and responsive care.
Help reduce fragmentation and barriers within the healthcare system.
Provide advanced clinical leadership across multidisciplinary services.
Work collaboratively with the Aboriginal and Torres Strait Islander Health Directorate and Cultural Coordination Team.
Build connections with the Institute for Urban Indigenous Health, Mob Link, local Elders and Aboriginal and Torres Strait Islander community organisations.
Work autonomously while maintaining strong partnerships across clinical and community services.
What they are looking for The successful applicant will be an experienced Registered Nurse who brings: Advanced clinical capability and professional judgement.
Strong verbal, written and interpersonal communication skills.
The ability to navigate complex healthcare systems and connect patients with appropriate services and support.
Experience working collaboratively with patients, families, multidisciplinary teams and community organisations.
An understanding of the social determinants of health.
An understanding of Primary Health Care principles.
The confidence to work autonomously and provide clinical leadership.
A genuine commitment to improving health experiences and outcomes for Aboriginal and Torres Strait Islander peoples.
A respectful approach that recognises the diversity of Aboriginal and Torres Strait Islander cultures, communities and individual healthcare journeys.
A relevant postgraduate qualification, current enrolment in postgraduate study or previous experience in Aboriginal and Torres Strait Islander health will be highly regarded.
