Community Worker – Salisbury Heights SA Kogan People Solutions, recruiters, are assisting a South Australian NDIS-registered home care provider specialising in delivering support services to individuals living with disabilities.
Broadly the focus is on Support coordination, Disability Support and Client Empowerment.
Our client is now seeking a qualified and experienced Community Worker (Support Coordinator) who can make meaningful impacts in the lives of individuals and their families.
You will assist clients to build their confidence, maintain independence, and connect with local networks, services, and opportunities.
Salary: Negotiable in the range of $80k to $85k plus superannuation per annum.
Tenure: Fulltime and ongoing.
Location: Based inSalisbury Heights SA with travel required to client's homes as required.
Key Responsibilities include: Support clients to access social, educational, vocational, and recreational opportunities in their communities.
Develop a solid understanding of the nature and extent of difficulties of client needs by conducting interviews.
Assess client needs and support the delivery of individualised, person-centred care plans to promote independence and wellbeing.
Facilitate community participation, social inclusion, and access to appointments, services, and activities.
Advocate and raise awareness of client's needs in areas such as recreation, health, housing, employment, and other social services, while actively supporting their inclusion and participation within the broader community.
Deliver emotional support and capacity-building assistance, including support for clients with disability, ageing,



and mental health needs.
Maintain accurate and detailed case notes, documentation, and reports about client progress.
Collaborate with families, carers, and healthcare professionals while adhering to organisational policies, NDIS standards, and WHS requirements.
You should be able to demonstrate the following attributes and strengths to be considered: Highly relevant post-secondary qualifications (at least Diploma level) in a relevant discipline, or agreed substitute.
At least 1 or more years professional experience since acquiring your qualifications in a highly relevant environment of supporting individuals with disability, mental health, or other complex needs.
Possess (or willingness to obtain) current certifications and checks such as NDIS Worker Screening Check, Working with Children's Check, First Aid CPR Certificate, and other relevant certifications.
Sound knowledge of NDIS and person-centred practices.
Excellent communication and interpersonal skills.
Strong organisational, advocacy, and reporting skills.
Ability to work autonomously and collaboratively as part of a team.
Strong cultural awareness and commitment to diversity and inclusion.
Unrestricted SA driver's license.
You must have unrestricted full time Australian working rights to be considered.
