Key Responsibilities
Conduct functional assessments and develop personalised care plans
Recommend and coordinate home modifications and adaptive equipment
Provide training and guidance to clients, carers, and support staff
Work collaboratively with multidisciplinary teams to achieve client goals
Maintain accurate, timely documentation in line with industry standards

About You
Current registration with AHPRA as an Occupational Therapist
Experience in community or home care (preferred)
Strong interpersonal and communication skills
Ability to work independently and manage a varied caseload
Current driver's licence and reliable vehicle
