In this role, you will:
Build relationships with clients, understand their needs and goals, and act as their key contact.
Coordinate and monitor services to ensure clients receive appropriate, person-centred support.
Monitor changing needs, funding and safety, and arrange reviews or escalate concerns when required.
Maintain accurate records and ensure services meet Support at Home and organisational requirements.
Work with clients, families, staff and providers to support positive outcomes.

Essential Criteria:
Demonstrated success in a previous Care Partner or similar role
Working knowledge of the Support at Home program
Exceptional people skills
Passionate about going above and beyond for others
Outstanding oral and written communication skills
Exceptional organisational skills
Ability to multitask and adapt to any situation
Self-motivated and proactive
First aid certificate (current or willing to obtain)
Desirable Criteria:
Certificate III in Individual Support
