About The Role
This role plays a vital part in ensuring secure, timely, and effective discharge planning for patients.
Working as part of a multidisciplinary healthcare team, the Discharge Planner supports patients and their families by coordinating care, services, and resources to promote continuity of care following discharge.
This is a full time relieving role Monday to Friday.
Your main duties will include:
Assessing patient needs and discharge readinessCoordinating community services and support agenciesCollaborating with clinical staff, patients, and familiesAdvocating for patient-centred, safe discharge outcomes
About You
Current AHPRA registrationRelevant clinical knowledge, skills and experience developed in an acute care environmentWillingness to educate and provide advice to patients and nursing staffDemonstrated critical thinking,



problem solving and planning skillsDemonstrated advance communication and interpersonal skills relating to internal and external customersExperience in discharge planning
