Experienced NDIS Support Coordinator – Newcastle, Lake Macquarie & Maitland
Caring Arms Australia is looking for an experienced NDIS Support Coordinator (Coordinator of Supports) based in Newcastle, Lake Macquarie or the Maitland region.
Join a growing team supporting NDIS participants, including people with complex disability and psychosocial support needs, to navigate their plans, overcome barriers and work towards meaningful goals.
About the role
As a Support Coordinator, you will work directly with NDIS participants to understand their individual circumstances, implement and make effective use of their NDIS plans, coordinate appropriate supports, resolve service barriers and connect participants with funded, mainstream and community services.
We are looking for someone who already understands Support Coordination and can confidently manage a participant caseload while building strong relationships with participants, families, providers and other professionals. The ideal candidate will have a minimum of 1 year, and preferably 2 or more years, of qualified experience as a Support Coordinator
This is primarily a full-time position, however experienced candidates seeking part-time employment are also encouraged to apply and can discuss suitable arrangements during the recruitment process.
Location - Primarily Work From Home
Based in the Newcastle–Lake Macquarie–Maitland region, this role is primarily work-from-home with partial office attendance at our Wyong office, typically one to two days per week. You’ll stay connected with the team while working locally and supporting participants in your own region
Key responsibilities
- Manage a caseload of NDIS participants, including people with complex or changing support needs.
- Help participants understand and make effective use of their NDIS plans, budgets and available supports.
- Identify participant needs, goals and service gaps through regular discussions with participants, families and key stakeholders.
- Connect participants with suitable NDIS-funded, mainstream and community services across disability, health, housing, employment and social supports.
- Coordinate with providers, families, allied health professionals and other stakeholders to keep supports working effectively.
- Monitor plan utilisation, participant progress, service delivery and emerging risks, and respond when circumstances change.
- Advocate for participants and help resolve service issues, barriers or breakdowns where required.
- Maintain accurate case notes, participant records, reports and other documentation in line with NDIS and organisational requirements.
About you
- Minimum 1 year, preferably 2+ years’ experience in NDIS Support Coordination or a closely related role.
- Relevant qualification (preferred), such as a Diploma or TAFE qualification in Community Services, Disability, Social Work, Allied Health or a related field.
- Strong working knowledge of NDIS plans, funding, budgets, plan implementation and participant choice and control.
- Experience independently managing a Support Coordination caseload, including plan utilisation,
reviews and changing participant needs.
- Confidence supporting participants with complex disability and psychosocial support needs.
- Strong skills in provider coordination, problem solving, stakeholder engagement, case notes and NDIS reporting.
- Current NDIS Worker Screening Check, Working With Children Check, National Police Check, NDIS Worker Orientation Module (Quality, Safety and You) completion, Australian Driver Licence and reliable vehicle.
