About the Role
As a Social Worker at Feros Care, you'll support older Australians with complex psychosocial needs to live safely, independently and with confidence in their communities. This is a part time (3 days per week), 6-month fixed-term opportunity with the possibility of extension. Using a person-centred, trauma-informed approach, you'll assist clients to access appropriate services while addressing social, emotional and wellbeing challenges.
Working across Victoria, New South Wales and Queensland through virtual and face-to-face service delivery, you'll advocate for clients, coordinate supports and collaborate with multidisciplinary teams to deliver holistic outcomes that enhance wellbeing, quality of life and access to services.
What You'll Be Doing
- Deliver person-centred social work support, including assessment, advocacy, counselling and care coordination
- Support older Australians with complex psychosocial needs to improve wellbeing, safety and independence
- Conduct comprehensive psychosocial and risk assessments and develop tailored support plans
- Help clients navigate aged care, health, housing and community support services
- Advocate for client rights and equitable access to services and supports
- Identify and respond to safeguarding concerns, including abuse, neglect and exploitation
- Collaborate with multidisciplinary teams to deliver integrated and holistic care outcomes
- Build strong relationships with community providers and external stakeholders
- Contribute to care planning, service coordination and ongoing monitoring under the Support at Home program
- Maintain accurate documentation and ensure compliance with aged care legislation, standards and professional practice requirements
What You'll Bring
- Experience in community-based social work, ideally within aged care, health or human services
- Proven ability to support clients with complex psychosocial needs and safeguarding concerns
- Strong assessment, care planning and case management skills
- Confidence navigating services and advocating for positive client outcomes
- Excellent communication, relationship-building and stakeholder engagement skills
- Strong critical thinking and problem-solving skills, with the ability to work autonomously in a remote service delivery environment
- Eligibility for membership with the Australian Association of Social Workers (AASW) (desirable)
- Experience in care management under the Support at Home Program (desirable)
- Tertiary qualification in Social Work or a related discipline (desirable)
