St Vincent's Public Hospital Sydney is seeking a Social Worker to coordinate the Residential Aged Care Placement Service, supporting the multidisciplinary team to transfer patients to Residential Aged Care Facilities (RACFs) and improve patient outcomes.

You will maintain the weekly RACF vacancy list, perform psychosocial assessments, and support patients and families during transitions, while building robust links with RACFs and community resources.
