About the role
This position will suit someone who is confident working with participants with complex needs, enjoys building strong professional relationships, and is passionate about advocacy and achieving positive, participant-led outcomes. You will manage your own portfolio of participants while working closely with families, guardians, allied health professionals, service providers, community organisations, government agencies and the NDIA. The successful candidate will also contribute to the continued growth of Support Coordination services, with an expectation of generating approximately three new participant referrals per month through professional networking and relationship building. Key responsibilities
- Provide high-quality Level 2 Support Coordination and/or Level 3 Specialist Support Coordination
- Support participants to understand, implement and make effective use of their NDIS plans
- Work closely with participants, families and support networks to understand individual needs, goals and preferences
- Coordinate supports for participants with complex needs and multiple service providers
- Identify barriers, risks and gaps in supports and collaboratively develop practical solutions
- Build strong professional relationships with allied health professionals, service providers, guardians, government agencies and other stakeholders
- Support participants through plan reassessments, changes in circumstances and other NDIS processes
- Advocate for participants' rights, choices, independence and preferences
- Maintain accurate case notes, reports and documentation in accordance with NDIS requirements and EDS policies and procedures
- Build professional referral networks and contribute to the growth of EDS Support Coordination services
About you
- Demonstrated experience delivering Level 2 Support Coordination and/or Level 3 Specialist Support Coordination
- Experience supporting people with complex disabilities, psychosocial needs and/or complex support environments
- Strong knowledge and understanding of the NDIS and Australian disability sector
- Excellent communication and interpersonal skills
- Strong assessment, problem-solving and case management skills
- Confidence managing competing priorities and working autonomously
- A collaborative approach when working with multidisciplinary teams and external providers
- Excellent written documentation and report-writing skills
- Strong networking and relationship-building skills, with the ability to develop referral pathways
- A current driver's licence and access to a reliable vehicle
