FRONTIDA SUPPORT PTY LTD provides person-centred disability and community care services to NDIS participants, primarily within participants' homes and the community.
We are seeking an experienced Senior Disabled Carer to join our team on a permanent full-time basis.
This is a hands-on care position responsible for providing high-quality, person-centred support to NDIS participants in accordance with their individual support plans, needs, preferences and goals.
In addition to providing direct care, the Senior Disabled Carer will provide day-to-day guidance, mentoring and practical support to Disability Support Workers to promote consistent service delivery and high standards of participant care.
The successful applicant will be an experienced disability support professional who can work independently, respond appropriately to participants with diverse and complex support needs, and provide practical leadership to less experienced members of the care team.
The primary focus of the position remains the direct provision of disability care and support to participants.
Key Duties and Responsibilities
The successful applicant will be responsible for:
- Delivering person-centred care and support to NDIS participants in accordance with their individual support plans, assessed needs, preferences and personal goals;
- Assisting participants with personal care activities, including showering, dressing, grooming, toileting, personal hygiene and other daily living activities;
- Supporting participants with mobility, transfers, positioning and the safe use of mobility aids and other assistive equipment;
- Assisting with meal planning and preparation, feeding and other nutritional support according to participants' individual needs;
- Providing medication assistance in accordance with participants' support plans, organisational procedures and the employee's level of training and authority;
- Assisting participants with domestic activities and daily household tasks to support safe and independent living;
- Supporting participants to access the community, attend appointments and participate in social, recreational and other activities according to their individual goals;
- Supporting participants with physical, intellectual, psychosocial and/or complex support needs in accordance with their individual support arrangements;
- Implementing person-centred support plans and adapting day-to-day support according to participants' changing needs and circumstances;
- Monitoring participants' physical health, emotional wellbeing, behaviour and general condition and promptly reporting changes, concerns or incidents to the appropriate person;
- Identifying and responding appropriately to risks, incidents and changes in participant needs and escalating matters where further professional or clinical input is required;
- Promoting participant independence and supporting participants to develop or maintain daily living and community participation skills;
- Respecting and promoting participants' dignity, privacy, choice, cultural preferences, rights and inclusion in all aspects of service delivery;
- Providing day-to-day guidance, mentoring and practical support to Disability Support Workers to promote consistent and appropriate care practices;
- Supporting less experienced staff in understanding participant support plans, individual needs, workplace procedures and appropriate care practices;
- Assisting Disability Support Workers to develop their practical skills and confidence when supporting participants with diverse or complex needs;
- Monitoring the standard and consistency of care provided and raising service quality or participant safety concerns with management where appropriate;
- Maintaining accurate and timely participant documentation, including progress notes, care records, observations and incident reports;
- Liaising and communicating effectively with participants, their families and carers, allied health professionals, other support workers and external service providers to promote coordinated and consistent support;
- Working collaboratively with multidisciplinary teams to implement agreed support strategies and respond to participants' changing circumstances;
- Participating in staff training, quality improvement activities and continuous professional development;
- Maintaining knowledge and understanding of the NDIS Practice Standards, participant rights and person-centred care principles;
- Ensuring services are delivered in accordance with relevant NDIS requirements, FRONTIDA SUPPORT PTY LTD policies and procedures, workplace health and safety requirements and applicable legislation;
- Following appropriate infection control, safe manual handling, safeguarding and risk management procedures;
- Identifying opportunities to improve the quality, safety and consistency of participant support and communicating recommendations to management; and
- Performing other disability care and support duties consistent with the employee's qualifications, experience and level of responsibility as reasonably required.
Qualifications
The successful applicant must hold one of the following:
- Certificate III in Individual Support (Disability);
- Certificate IV in Disability Support, Individual Support or a related community services qualification; or
- An equivalent overseas qualification recognised as comparable in Australia.
Experience Requirements
The successful applicant must have:
- A minimum of two years' full time, or equivalent part-time, relevant experience providing disability support or community care services;
- Demonstrated experience supporting people with physical, intellectual, psychosocial and/or complex support needs;
- Experience implementing person-centred support plans and promoting participant independence;
- Demonstrated experience assisting participants with personal care, community access, medication assistance and daily living activities;
- Experience monitoring participant wellbeing and responding appropriately to changes in individual needs or circumstances;
- Experience maintaining accurate client records,
progress notes and incident reports;
- Experience working collaboratively with participants, families, allied health professionals and multidisciplinary care teams; and
- Demonstrated ability to work independently, manage competing priorities and respond effectively in a busy care environment.
Previous experience mentoring, guiding or supporting junior Disability Support Workers is highly desirable.
Mandatory Requirements
The successful applicant must also hold or be able to maintain, as applicable:
- Current First Aid and CPR certification;
- A valid Working with Children Check, where required;
- A current NDIS Worker Screening Check; and
- Any other mandatory clearances or checks required to undertake disability support work for FRONTIDA SUPPORT PTY LTD.
Required Skills and Competencies
The successful applicant must demonstrate:
- Strong experience in providing person-centred disability support;
- A sound understanding of the NDIS, NDIS Practice Standards, participant rights and person-centred practice;
- Strong communication and interpersonal skills and the ability to communicate effectively with participants, families, colleagues and healthcare professionals;
- The ability to provide practical leadership, guidance and mentoring to other Disability Support Workers;
- The ability to understand and implement individual support plans;
- The ability to identify changes in participant needs and respond appropriately to changing circumstances;
- Strong organisational, time-management and problem-solving skills;
- The ability to maintain accurate, timely and confidential records and documentation;
- Knowledge of workplace health and safety, infection control, safeguarding and secure manual handling practices;
- The ability to work independently while also contributing effectively as part of a multidisciplinary team;
- A qualified, patient and respectful approach when supporting participants with diverse and complex needs; and
- A strong commitment to promoting participant dignity, independence, inclusion, choice and quality of life.
Employment Conditions
This is a permanent full-time position working 38 hours per week.
The current base rate is SCHADS Award Level 2, Pay Point 1 – $36.22 per hour, equivalent to approximately $71,577.48 per annum, plus superannuation, based on a standard 38-hour working week.
As FRONTIDA SUPPORT PTY LTD provides home and community-based support services, the successful applicant will be required to provide services at participants' homes and other community locations across Sydney according to participant support requirements and the operational needs of the business.
The employee may be required to work across different shifts and locations and must comply with all FRONTIDA SUPPORT PTY LTD policies and procedures and all applicable NDIS, workplace health and safety, safeguarding, privacy and other regulatory requirements.
